Put the form and an unsealed self-addressed stamped envelope inside a separate stamped envelope. Mail it to March of Nickels, 258 N Main St., West Bend, WI 53095 during the Promotion Period. We’ll mail your location-specific QR-code entry back in the enclosed envelope.
